 The first thing to fix is money stress on doctors. In this fantasy system doctors would be paid ten times the average USA income. In 2024 the average was $67K, so doctors would be paid $670,000 per year. They could make money "on the side" from people willing to pay more. The government would pay the base salary of $670K. This decouples the doctors from basic money stress, and let's them focus on people and medicine. Smarter doctors could loaf some and other doctors would work hard to keep up.

 Also, there would be more mistakes made, because the system would only spend a reasonable amount of time on each person. If you were sick all the time, it could be time to go back to Jesus, no more doctor visits. The press would scream that the system is cold and uncaring, but actually the opposite would be true for reasonably healthy people. The sickies would get less care. 

There would be no malpractice, doctors could be tried as criminals if the case warrants it.

Doctors would go to required therapy sessions to discuss things. They would take an annual psychological evaluation, and get help if they were outside of norms. Some doctors would retire early and get less retirement. Being a doctor is stressful, sick people are very stressful.
